Add more options to the registration page

Requirements

Add the following options in the page:

Are you enrolled in any kind of education
Options: 1- Yes
2- No
If yes, please specify:
Options: 1- Formal Education
2- Non-formal Education

In case they select option 2, can we have an option of them writing the name of the non-formal educational institution?

Are you taking part in one of the Breteau Foundation projects?
Option 1: No
Option 2: Breteau Foundation Mobile Education Bus
Option 3: Breteau Foundation Mini Tented School
Option 4: Breteau Foundation Partner

These data need to be saved into the database
