Sub Issue: A Button for Additional Information Form

Description

This issue is about creating a button that links to a form for the user to fill out more information about the students. This will include emergency contact information, phone numbers, and allergies.

This issue just focuses on creating the button that shows up on the main page in the left sidebar. The button as of now will lead to a non-existing page.

The file in the front-end repo for adding the button will be templates/partials/sidebar-left.tpl
