Background
We have a new PD feedback form. How are we going to communicate it to cohors?

Acceptance criteria

  • define how it'll be presented and communicated to volunteers
  • add it to canvas

Posted feedback form to both NW6 and WM5 and updated Canvas for WM5. However, NW6 Canvas is readonly for me so I can't update it. I've requested access.
