Add "app" and "staging" subdomains

we need to add "app" and "staging" as subdomains that shouldn't automatically redirect to /map. That should help fix some of the issues we are having with the testing server. And while we're at it, can remove "staging-cigrp" as that old subdomain is no longer used.
