Already brought this "issue" to coding/mapping team. Chronic cardiac disease can be mapped but the question on the CRF clearly excludes hypertension. Only with SNOMED-CT, as far as we know, could a complex coding be generated. After some checks we finally came up with the following exact match using post processing of SNOMED CT:
128238001 |Chronic heart disease (disorder)| : 315215002 | Disorder excluded (situation) |= 38341003 | Hypertensive disorder, systemic arterial (disorder)|
Or dependend on parsing: 128238001 |Chronic heart disease (disorder)| + (
315215002 |Disorder excluded (situation)| :
246090004 |Associated finding (attribute)| =
38341003 |Hypertensive disorder, systemic arterial (disorder)|)

So we can state we have an exact match (once the semantic model has been adjusted/improved/made more specific)?

for now only Chronic cardiac disease is mapped.
