Request: Observation-cause-of-death-part1 example with multiple causes

I have a request for an additional example for the Observation - Cause of Death Part 1 resource. Often you will see multiple causes listed under the first cause of death since many localities let you list up to four. It would be great to have an example which included more than one cause of death to display how the line numbers and grouping works (note: I am not referencing a contributing cause of death (i.e., cause of death part 2)).
