Teamwork task: Upgrading colony version is incorrect + broken link

Description

This includes both a couple of bugs and some enhancements to the Upgrade colony action to help provide users with more context to upgrades and to be more accurate with the details.

Steps to reproduce

1. Have a colony that is 2 version lower then the latest version of Colony Network.
2. Create a new Upgrade colony action.
3. Note that the UI will show that the upgrade will be from version 2 higher, however, this won't actually be the transaction performed.

Enhancements

• There should be an a info banner when viewing the action if there is a new version and if they are on the latest version. This banner should be viewable to everyone.

  • The URL for the "View release notes" for each case should be: https://github.com/JoinColony/colonyNetwork/releases

• For completed Upgrade colony actions, the UI should be updated so that the "Current version" now says "Previous version". The notice banner should also be updated so that it is accurate. Expected behaviour

• Colony version should be accurate to the upgrade happening.
• The 'Learn more' link in the calamity banner should link to the Colony Network release notes
  • URL: https://github.com/JoinColony/colonyNetwork/releases

Actual behaviour

• Incorrect version when upgrading a colony. The version shows that the upgrade will be 2 versions higher, however will only upgrade one version.
• The learn more link in the header goes to a broken link, where the docs have been removed.
• After upgrading a colony and the action is complete, the UI says that the "Current version" is the old version, which is not correct.
