Add functions "forgot password" and "forgot username" #111

On login page, offer these links:

"Forgot password"

new page asks for user name
will send email to address provided in account
email offers link to (generate automatically? or) set a new password

"Forgot username"

new page asks for email
will send username to email provided in account

For security matters, do not inform if the entered username or email were recognized at all

Kebap added the enhancement label
