• ocrd_utils.make_file_id: only fall back to output fileGrp + (page)ID instead of page counter, fix make_file_id #861
• OcrdWorkspace.add_file: when ID already exists, remove (with overwrite) or fail instead of reusing
• Workspace.merge: delegate force to each add_file
