Move era table scripts to GitHub, make them SQL-dialect specific? #551

People should always run the era scripts when ETL-ing (right?), but the scripts are currently here in OHDSISql.

Maybe it would be better to provide them in the dialect-specific GitHub folders instead, so for example for Postrges put the script here?
