install.sql has these lines:

INSERT IGNORE INTO `dialplaninjection_module` VALUES ('modulerawname', 'dialplaninjection');
INSERT IGNORE INTO `dialplaninjection_module` VALUES ('moduledisplayname', 'Dialplan Injection');
INSERT IGNORE INTO `dialplaninjection_module` VALUES ('moduleversion', '0.1.1m');

Don't know why these details are stored in MySQL, they can conflict with settings in module.xml
