On GitHub.com, navigate to the main page of the repository. Under your repository name, click Settings. In the "Features" section, under "Issues," click Set up templates. Use the Add template drop-down menu, and click on the type of template you'd like to create.

Use the template that is in the contributing.md file for issues and PRs
