For better gender-neutral and order of the options:

Faço a minha rotina habitual
Presumo estar saudável e estou em casa, por prevenção minha
Sou um caso suspeito e estou preventivamente em afastamento social para prevenir potenciais contágios
Sou um caso positivo e mantenho isolamento, para não contagiar outros cidadãos
