Users with both username+email address apply for membership in collaborations twice

As a user with both a username and email address, apply for membership in a collaboration.

Collaboration administrator sees "Review 2 requests" appear in collaboration's box at /collaborationList/
At /collaboration-review/collaborationName , requests are present for both fields and can be responded to independently.

Expected: Only one request generated. Suggested format: username ([email protected])

also, refreshUserProfileCollaborations isn't being called when a collaboration administrator approves a collaboration request.

Repro:

Blake "Apply for membership" in restricted collaboration AA owned by Alex.
Alex sees "review 2 requests", Blake and Blake@email.
Alex "Approved" for both, "done".

Under AA: Alex sees: Blake & blake@email now listed as collaborators.
Under AA: Blake sees: "Apply for membership", Blake listed as collaborator (NOT blake@email)

Blake does not see AA's posts in Posts For All Of Your Collaborations.

Then. Blake "Join this collaboration" or "Leave this collaboration" in another, unrestricted, collaboration.

Under AA: Blake now sees: "Leave this collaboration"

Blake sees AA's post in Posts For All Of Your Collaborations.
