Would you be happy if I created a Umbraco Forms plugin for use with this?

Sure! I do have a workflow for Umbraco Forms you can use as a starting point if you like. It needs some tweaking but I haven't had a chance to get to it.

For example, it would be nice to be able to attach uploads, use a template, etc. Also needs to be updated for Umbraco 9 and 10.
