Hi, I'm having a problem with the v1.04 release on my Windows 7 64-bit machine. No icons are displaying, and when I click where I believe the "open file" button is based on the screenshot from the README, the program crashes.
What I did
I downloaded the v1.04 release (as that was the release with Windows binaries)
I extracted the files to C:\Users\Sam Cantrell\bin\pdftag
I ran pdftag.exe
The program opens, with no program icons shown except what appears to be a generic icon.
I click what I think is the "open file" button, as I don't see any tooltips.
The program crashes.

Sorry for the late reply.
The Windows version has not thoroughly been tested... But I just tried on a Windows 10 64 bit and it works as expected. However, on my machine it is not using Adwaita theme.... Have you, by mistake, deleted the shared and etc directories that come with the Windows app? Because that is exactly what happens when it can't find those directories. The shared directory contains the icons and the etc directory tells the app to use the win32 theme. If you want the Adwaita theme, you can remove the etc directory, but the shared directory must be there for the icons to appear.
This is how it should look like after downloading and decompressing the .exe file and running the pdftag.exe:
