This improvement should allow code like:

CombinatoricIterator<Integer[]> iterator = factory.createCombinations(3, 1, 2, 2, 3, 3, 4);
iterator.nextIndex(5);
Integer[] sixthElement = iterator.next(); // zero indexed.
