Data model cleanup

Around the time of The Great Big Payment Migration, we made some changes to the User model (and maybe others) that made certain fields defunct (email, first_name, last_name, account_disabled, etc).

They should be easy to remove, but we need to be careful and search through the code to ensure no lingering dependencies.
