Title page and footer in CDM pdf #25

The pdf of the CDM still has a date of 21/08/2017 & ref of C3S_D311a_Lot2.2.1.1_201708_Initial_specification_for_CDM_v1

Good to also include the version of the CDM as part of the header page.

Also "C3S_311A_Lot2_NUIM_2017SC1 - Initial specification for CDM" in the footer.

Needs edits to https://github.com/glamod/common_data_model/tex/cdm.tex
