Optionally support collecting other client demographics, such as Ethnicity

As a food bank admin, I want to specify my own demographic fields, such as Ethnicity or Language, that would appear on the household screen and fill text fields on the Household record.

Questions:

• Would these be picklists? Would we allow them to specify the options, or would we get them from the actual field describe info?
• Would these be dynamic? Or can we come up with a list of fields to support, and just let them turn those on or off and specify options available for each?
• Would ethnicity or other demographics need to be per client as well? Might be tricky to include them in the UI.
