Capturing operational notes

In addition to the notes that are found in the index table, there are also notes in the op_notes table. These notes capture aspects of the document that can help determine who should be assigned the document. It also captures assignment info that doesn't fit into the CE/PE/RE/PUB roles such as reference checking, in depth IANA review, and/or formatting.
