Sometimes it goes for weeks perfectly fine, but occasionally the add-on just doesn't convert into symbols when using edit during review, and I have to use the external editor or browser to generate the symbols.

Did you try closing the add window and see if it fixes the issue? Or closing all windows except the main window?

What version of anki are you using?
