Last time I used Google docs and worked fine. The output generated was a CSV with each row corresponding to an entry.

@lindenb also suggested survey monkey but I've never tried this.

I think we should use google docs to collect the data. There is an interesting library to deploy the survey on your own domain using heroku. I think we could create the survey in google docs then create a webpage with the survey on it. We could have a custom domain for the survey eg. bioinfsurvey.com or something similar. This is nicer than the very long urls generated by google docs.
