Update 12/16

A new repository has been created for the early access. Please go to Azure Monitor OpenTelemetry Profiler for .NET repository for the instructions of how to access the new package alone with a Get Started in the readme.

There are several beta versions there, it is recommended to use 1.0.0-beta4 or newer.

Please feel free to file issues in the new repository for questions, feedback and suggestions.
