Upload feature failed for some users

Hi there,

I'm facing this strage error when trying to upload a documents using my account A; however, I used account B to upload and it works fine. Couldn't find what's wrong with it.

Looked into this one with @Keatnuxsuo -- the root cause was because the embedding model token limit was set too low.
Suggest 2 review items here:

• better error handling and messaging to the user to help identify root cause
• review of default token rate settings (e.g. gpt-4o is set to 30k right now which is very low) -- though if the error messaging is good, then we could keep it low for compatibility with quotas and indicate clearly to users/devs what needs to be done to address
