The ability to handle checkboxes in text

Hi all,

We have a use case where a dermatologist is filling in a form with checkboxes. The checkboxes will look like:
[ ] - Full body skin exam
[ ] - Upper body skin exam
[X] - Lower body skin exam
[ ] - Focused skin exam of _

The current version will assert all 4 types of exams where just the Lower body exam should be asserted.

I has been suggested to use Azure Forms Recognizer which has advanced OCR capabilities including the ability to detect checkboxes. Has anyone does this or know what it would take?
