Is your feature request related to a problem? Please describe.

I'm always frustrated when I want to add a new test case request and do not have any template for it.

Describe the solution you'd like

Add a repo-specific template for the test case requests.

Describe alternatives you've considered

Removing the existing template and pushing my imagination to its highest levels every time.

Additional context

I did it some time ago: #448, #696.
