Properly addressing both issues properly probably involves moving to a vendor media type so we don't break old clients, unless we make a clean break for Annotator v2.
I've been checking out http://jsonapi.org/ a bit and think it might be very sensible for an annotation store to implement.

As per openannotation/annotator-store#66, I think we should use RFC6570 URI Templates in the Store plugin's configuration.
