Can not download any record.

Bug description:
No record can be downloaded.

Steps to reproduce:

1. Start to declare a notification as Health worker and send it for review.
2. As a reg agent, try to download that record to review.

Actual result:
Fails to download the record.

Expected result:
User should be able to download the record for further process.

@onnee04 sorry about that. The issue is resolved now. But I had to clear all data on the QA environment to fix it. At least you should be able to continue now
