Fetal death field missing in death declaration form.

Bug description:
Fetal death field missing in death declaration form.

Steps to reproduce:

1. Start to declare a death record as Health worker/ reg agent/ registrar
2. Land on Event details page.
3. Check the "Cause of death has been established" checkbox.

Actual result:
Fetal death field missing after date of death field.

Expected result:
Fetal death should be available after date of death field.

@jpye-finch I guess this was missed in the original installation. Niue has been live with this field not existing. As the country hasnt raised this, I suggest the Figma file is updated and this bug is closed.
