Design a better feedback form

Possible enhancements include:

1. Add email field.
2. Collect emails on the server.
3. Evaluate form validation and improve as needed.
4. Evaluate form security and improve as needed.

Instead of keeping the WuFoo form, start over with a React component that lets people submit ideas and vote for them. Or instead of submitting ideas, maybe just present people with a static set of budget-related issues? What do you think, @villatrue ?
