As a data collector, I would like to be able to looking up referrals using a QR code or other searching methods. This is the ability to tie child data collected at the structure to additional data collected at the health facility on that child due to referral. Once a referral is made at the structure, a QR code is assigned/associated with that referral (paper referral form) and the HFW also takes a photo of the QR code from the form at the HF. If the QR code gets lost along the way, there is also a look-up function for children that have been referred, which queries based on referral=yes and returns a listing of names which can be typed into a search box.

Acceptance criteria.

  • Be able to scan QR code on Dispense SMC or SPAQ redose if the answer to question “Was the child referred to the HF for any reason?” is “yes”.

  • Be able to scan QR code on the HFW HF-level referral form to link the referral form to children that were referred.

  • on the HFW HF-level referral form, populate the fields for child information with data for the associated child after the QR code scanning if data has synced, or if not synced then allow for users to input those details.
