A feedback form should be made available to allow users to contribute feedback

The current plan is to embed a Google Form - please note that this will probably store cookies and collect user information so will have implications for the site's privacy and cookie policy (#10)

@sylvhay I have added a link to your privacy policy page to the "Agreement checkbox".

Fortunately, it appears to open in a new tab so that users can read the policy without navigating from the form and risk losing their entries.

Unfortunately, I do not think Google Forms allows setting an anchor (i.e. make "Privacy Policy" the link).

Also, should there also be a checkbox for "Terms of Use"?

Thanks @tmfrnz , I have just added 'and Terms of Use' to the same checkbox to make it easy, not sure if it needs to be there but cant hurt?
Annoying that it wont allow setting an anchor as it looks a bit awkward (esp. with the two links), but I guess it doesnt matter. Can change to two separate checkboxes if you think it would look better on the page.

@sylvhay how about moving the text to the description and shortening the checkbox label to "I agree" (or similar)? We think it looks a bit cleaner.

Further, I marked that field as required.
