What is the name you gave to PDFAnnotator instance? Send us a screenshot of the general settings of your PDFANnotator instance
We have not been able to recreate this issue with v1.5.5 and v1.5.7 in our Moodle4.3 and Moodle 4.5 instances

I meant the Settings page of PDFAnnotator instance in your course. The name you put here in the 'Name' field will show up as the title of the tab(e.g: for the instance in the picture it will be 'bjkhk')

It will also help if you can list out the steps to recreate the problem
