HTML to Markdown table conversion: how to generate empty table headers?

In order to convert HTML Javadoc into VS Code-compatible Markdown, I'd like to change the table rendering so that, if an HTML table doesn't have an header row or thead, an empty one would be generated instead.

So this:

    <tr><td> Abbreviation  </td><td> <code>.abbreviation</code> </td><td> <code>*[]:</code></td></tr>
    <tr><td> Code fence    </td><td> <code>.codefence</code>    </td><td> ``` ... ```                                       </td></tr>
    <tr><td> Explicit link </td><td> <code>.link</code>         </td><td> <code>[]()</code>                                                  </td></tr>
</table>  

would be rendered as:

|---------------|-----------------|-------------------|
| Abbreviation  | `.abbreviation` | `*[]:`            |
| Code fence    | `.codefence`    | \`\`\` ... \`\`\` |
| Explicit link | `.link`         | `[]()`            |

I have looked at the custom HtmlNodeRenderer example but I haven't figured it out so far.
