As per this feature request:

Choosing the type of feedback could be nice (e.g. problem, suggestion, idea, praise)

I like the idea. A simple dropdown could work. In the back-end it could be a custom taxonomy (if requests are stored as posts, see #4)

A simple dropdown could work.

Radio buttons with icons next to the labels could work too.
