Strange. I have an topic for this, via Wiki of the plugin repo - https://wordpress.org/support/topic/menu-options-values-lost-upon-upgrade/#post-7147440
Maybe this helps one.
However it is strange that you have the possibility to add custom options - works? But no checkboxes.
Can you add an another screenshot, before open the dev-tools, F12, and select the item on a checkbox, like my screenshot with Ctrl-Shift-C, or icon left in the top bar of the dev-tools to inspect the checkbox.

thank you so much for such an instant reply, and thank you for sharing your work!

I was keeping testing and I discovered that the issue is linked to custom user roles, please watch this video
https://www.youtube.com/watch?v=Y9zx4lSYnvQ

here's the screenshot you asked for
http://it.tinypic.com/r/nmaqz7/9

at first time I haven't tested standard users roles since I only have to disable elements for custom user roles, sorry if I wasn't accurate from the beginning.

do you want me to check something in the db? or do you want admin access? may I help you with something?
thank you again
