Incorporate pull requests CooperRS#8, CooperRS#9, and CooperRS#11 from original decrypt-otpauth-files, plus some additional edits.

First of all, thanks for the great app!
I want to store my backups on a non-digital medium, so I've added a
--pdf-outoption which allows specification of a PDF output file containing the same descriptions and QR codes currently displayed on the terminal.Unsure whether this is something you'd want to support, but here's a PR for it anyway.
