From Simplifier issue #3307

As part of the work being undertaken by the Pathology Standards and Implementation team, a requirement to add a new value to the UKCore-Observation-Lab.interpretation ValueSet has been identified.
The required value is:

  • Code: PA
  • Display: Potentially abnormal

The value is currently available as part of the existing PMIP EDIFACT (NHS003) messaging specification. PMIP EDIFACT (NHS003) is planned to be replaced by the FHIR profiles described in the Pathology FHIR Implementation Guide.

The addition of the new value to the UKCore-Observation-Lab.interpretation ValueSet will facilitate the transition from PMIP EDIFACT (NHS003).

Plan

  • Review by phil to ensure what has been produced is correct. @p-brennan Would this be used in other nations? Does this code have a finite life, i,e. once people have transistion to FHIR / REST it will be removed?
  • Review by DHCW to ensure fit for Wales
  • add to STU3 to be reviewed at a later date (ballot 3)

For context, the following table lists the existing four values associated with the PMIP EDIFACT NHS003 Result Normalcy Indicator. As shown, three of the EDIFACT codes (HI, LO and OR) have a direct mapping to the existing Observation.interpretation value set.

The subject of this change is the addition of a new Observation.interpretation value set code for PA (Potentially abnormal).

Please note that the mappings are awaiting review by the relevant laboratory and GP system suppliers.

EDIFACT Code EDIFACT Description FHIR Code FHIR Description FHIR Definition
HI Above high reference limit H High The result for a quantitative observation is above the upper limit of the reference range (as defined for the respective test procedure). Synonym: Above high normal
LO Below low reference limit L Low The result for a quantitative observation is below the lower limit of the reference range (as defined for the respective test procedure). Synonym: Below low normal
OR Outside reference range A Abnormal The result or observation value is outside the reference range or expected norm (as defined for the respective test procedure). [Note: Typically applies to non-numeric results.]
PA Potentially abnormal PA Potentially abnormal TBC
