Site: Remove committer by request process

The committer by request process was introduced after a discussion [1] in the dev list.

However, internal discussions on private@calcite showed that not everyone in the PMC is fully inline with this special process.

Moreover, the process is against the notion of becoming committer based on "merit" [2].

After receiving your email the PMC will evaluate your request and get back to you in 1-2 weeks
(usually a vote for adding a new committer takes ~7days).
