Update to Ruby version 2.1.
Remove ruby-switch as it is no longer required and isn't available in Ubuntu
14.04 LTS anyway.
Actually run apt-get update if required. Fixes the 1.2.0 fix.
Show build status as SVG in README.
Tweak PostgreSQL integration test.
