This also amends the `create-entry` script to always ensure that it puts the files under ``$ROOT/changes`, where `$ROOT` is the root of the `chainweb-node` source.

This should be much easier and much more fullproof. As a bonus, we can stop doing the "Internal changes" section because the new
Commitssection will include everything anyway, leaving only user-visible entries in theChangessection.
