Rather than being "NHS.UK prototype kit", the service name should be something which is more obviously something that you should change to be the name of your service.

Following #419, this updates the default index page to give clearer guidance on how to: * set the service name (which is now set to "Service name goes here" by default) * edit the index page * use the external links and included example templates Inspired by the GOV.UK Prototype kit default index page.
