remove gof_cancer rule from default logic but allow it if it exists #64

Before inclusion of the multistage cancer/multitumor model, a model recommendation rule existed to optionally set the goodness of fit threshold to 0.05 instead of 0.1 for cancer datasets. Remove this option from the default since it is no longer needed; but allow it to be optionally specified for backwards compatibility of existing datasets.

Hide row if not available, but show if it exists for backwards compatibility: USEPA/BMDS#60
