https://app.o2o.com/events/oR1M6g62zj/forms/new	New form can be created under registration page, in order to use the created form it needs to be assign as the default form of the event	To create new form, go to Event Manage > Setup > Registration > click Add forms > filled up those fields needed and publish or saved	- new form will be saved and will appear on the form list, also this can be use to attached on the event	---
